Library Permission Form

Hello 11th Graders,

I have sent your parents a library permission form to complete giving you permission to walk to the library. Traffic to the library has increased and I want to make sure you have permission. Additionally, it has come to my attention that some students have been reported visiting other locations around the hospital and not in the library. You should not be in any other spots in the hospital (including the cafeteria). If you are found in other parts of the hospital, you can receive a major office referral for being out of area. Ask your parents to sign the parent square form they received Sunday. You need a permission form on file before you can go to the library starting Monday, September 29.
